Northwest Senior Care has developed a range of offerings and resources to best prepare you for the road ahead. Originally providing only home care services, soon it was realized that much more should be offered to the seniors of Seattle, Bellevue, and the surrounding King County area. One difficult task families often face is that of deciding which type of senior facility would best suit their elderly loved one. This challenging decision inspired our Free Senior Housing Referral program, and ultimately, a passion to seek out other new and innovative avenues for providing valuable services to seniors. A hybrid offering, for example, which pairs monitoring technology with traditional care services has allowed us to reduce the out-of-pocket costs for our elderly members. Lively ImageThe ever advancing technology industry inspired Northwest Senior Care to develop a hybrid service using non-invasive monitoring technology developed by Lively. By utilizing the technology jointly with traditional care services we can accomplish much of the protective supervision that the senior requires from afar. There are several benefits to this approach, but perhaps most valued is the reduced overall cost felt by senior.

With the monitoring technology being an optional addition offered at no extra cost, we are often able to recommend that the senior require less of the more expensive hands-on care services. With the activity data being available  to the family via smartphone, tablet, or desktop, they are able to be sure their aging loved one is living safely at home and receiving quality care services.

Most incidents such as these occur within the home and can be prevented by taking simple precautionary steps. In an attempt to ensure that every senior has the opportunity to remain safe while living at home, Northwest Senior Care offers free home safety checks to any senior who may need one.

By ensuring that certain basic safety measures are in place, the level of injury risk drops significantly. We believe that having access to a home safety inspection by experienced care specialists is every seniors’ right. A misplaced throw rug, or an improperly lit bedroom can be the difference between a living safe at home for years to come and an unnecessary hospital visit.

Senior transition service1 in every 6 seniors who are released from the hospital will end up returning within 30 days. Decades of research on why this issue is so frequent in the elderly population has uncovered several recurring factors linked to the problem. By using this information, Northwest Senior Care was able to develop the Trusted Transitions hospital discharge assistance program.

Verifying with the seniors pharmacist that there are no conflicts of new and old medications, testing the understanding of the discharge plan with the elderly patient, and establishing a calendar complete with follow-up appointments, expected recovery progress, and activity benchmarks are just a few of the steps taken to ensure a safe transition home. It is our belief that if we implement the steps needed to avoid costly readmissions, then our elderly citizens will benefit both physically and financially.
